I usually use TDK CDRs, TDK or SONY MiniDiscs and type II tapes eg. Maxell XL-II or Sony UX-S.
Please, no high speed dubbing when recording tapes.
I create and print j-cards with the setlists. If you do the same that's great, if not don't worry, just enclose the setlist - can be handwritten.
I usually enclose the cases, but it's not the must.
